Success: Movement from Incarceration (SMI)

A specialized team of clinicians and support staff who work with clients for 18 months, serving individuals who come out of Santa Rita jail primarily and returning residents across all of Alameda County who are experiencing severe and persistent behavioral health challenges.

Felton’s Success: Movement from Incarceration (SMI) program is comprised of a specialized team of clinicians and support staff that work with clients to address serious mental health needs.

This program helps increase coordination among its service delivery systems; supports clients in navigating the transition of care to improve client wellness and community functioning, expands the interdisciplinary treatment team approach of planning of care to connect clients with short and long-term supportive services, and improves client outcomes to reduce recidivism and promote self-efficacy.
