Success: Movement from Incarceration (SMI)

A specialized team of clinicians and support staff who work with clients for 18 months, serving individuals who come out of Santa Rita jail primarily and returning residents across all of Alameda County who are experiencing severe and persistent behavioral health challenges.

Success: Movement from Incarceration (SMI) is a Felton Institute Justice Service Program that provides re-entry services in collaboration with the Alameda County Probation Department.

Felton’s SMI program is comprised of a specialized team of clinicians and support staff that work with clients to address serious mental health needs.

This program helps increase coordination among its service delivery systems; supports clients in navigating the transition of care to improve client wellness and community functioning, expands the interdisciplinary treatment team approach of planning of care to connect clients with short and long-term supportive services, and improves client outcomes to reduce recidivism and promote self-efficacy.

Formally and Currently Incarcerated Individuals: We are here to partner with you so that your journey becomes easier.

Linkages to Housing: Successful re-entry is not possible without a place to live.

Correctional/Re-entry Professionals: We are here to collaborate with you to support successful community reentry.

For Family and Support Networks: We provide group and individual interventions for successful reunification.

For Potential Employers: We provide coaching, mentorship, and life skills trainings to our participants to equip them with effective communication and positive work ethics.

For Community: We provide a safe, communal village that fosters growth, connection, diversity, and most of all love.
