Our Intake Process

If you think (re)MIND® might be the right place for you or someone you know, call us and we’ll help guide you through a simple assessment process.

For Individuals

Calling on behalf of yourself

Step 1 – Call your local (re)MIND® Program. When you call, you will complete a phone screening with one of our staff members.

Step 2 – Once residency and insurance are confirmed, you will be scheduled to complete an in-person comprehensive assessment with an assigned staff.

Step 3 – After the assessment is complete, you and your identified family/support network will be invited to attend a feedback session where our staff will provide education about the results of the assessment and treatment recommendations. If it is determined that other services are more appropriate, staff will provide referrals to the appropriate provider.

Step 4 – If it is determined that (re)MIND® services are most appropriate for you, you will be enrolled into full program service and collaboratively develop a Plan of Care, which will include access to the full array of services provided at (re)MIND®. You will remain eligible for ongoing services with your (re)MIND® team for up to two years.

For Clinicians

Calling on behalf of a young person

Step 1 – Call your local (re)MIND® Program. When you call, you will complete a phone screening with one of our staff members.

Step 2 – Discuss the referral with your young person and/or their family and…

  • If already diagnosed, discuss treatment options;
  • If possibly high-risk, discuss evaluation and treatment options;
  • Provide brochures, handouts, and use screening questionnaire;
  • Obtain consent to exchange information.

Step 3 – Schedule an appointment with (re)MIND® staff to meet one-on-one with young person and/or their family or have them call Felton Early Psychosis Programs to complete the phone screen.

Step 4 – The referred individual will complete an in-person diagnostic assessment. Additional assessment information may be gathered from family as appropriate.

Step 5 – (re)MIND® staff will request supporting collateral documentation from previous providers.

Step 6 – After the assessment is complete, the young person and their identified family/support network will be invited to attend a feedback session where our staff will provide education about the results of the assessment and treatment recommendations. If it is determined that other services are more appropriate, staff will provide referrals to the appropriate provider.

Step 7 – If it is determined that (re)MIND® services are most appropriate, the young person will be enrolled into full program service and collaboratively develop a Plan of Care, which will include access to the full array of services provided at (re)MIND®. Cost

Each (re)MIND® location offers services at no charge for county residents that meet eligibility requirements, varying by county.

San Francisco can serve a limited number of out-of-county residents on a sliding scale basis.

Insurance Requirements

All (re)MIND® programs serve individuals with Medi-Cal Insurance.

San Francisco and San Mateo (re)MIND® Programs also serve individuals without Medi-Cal. The Monterey (re)MIND® Program can serve a limited number of individuals without Medi-Cal.

Resident Requirements

Each (re)MIND® Program serves local residents within the county of operation. (re)MIND® Programs are located in five counties: Alameda, Marin, Monterey, San Francisco, and San Mateo.

San Francisco can serve a limited number of out-of-county residents on a sliding scale basis.

How are the Felton Early Psychosis Programs funded?

The Felton Early Psychosis Programs are funded by the statewide Mental Health Services Act (MHSA), passed in 2004 to reform California’s public mental health system to provide those from at-risk, underserved and unserved communities free public mental health services.
