
Located in Sonoma County, this 80-bed transitional recovery and community re-entry program offers 6 to 12 months of interim shelter and support services for individuals who are unsheltered and experiencing mental health challenges. BHBH provides a healing, recovery-oriented atmosphere for those who may also be addressing co-occurring substance use issues.
How Does BHBH Support Residents?
BHBH is designed to stabilize residents’ living situations while offering continuous onsite support to promote recovery and prepare for long-term housing.
The program uses a multidisciplinary team approach to meet each resident’s needs. Staff work closely with clients to develop individualized recovery plans that include personal goal setting, relevant education, and connections to community-based resources.
Through group wellness activities and mental health services, BHBH provides a structured yet compassionate foundation for sustained recovery.
The program is staffed 24/7, offering consistent support and care throughout each resident’s stay.

Individuals must meet all of the following criteria:
- Have a mental health condition that requires treatment
- Be between the ages of 18 and 65
- Be currently unsheltered or insecurely housed
- Be in the Sonoma or Marin County Behavioral Health system
In addition, individuals must also meet one or more of the following background criteria:
- In the process of being discharged from one of the following:
- A correctional facility
- A locked long-term care (LTC) facility
- A crisis service such as Crestwood’s Sonoma County Healing Center, a Crisis Stabilization Unit (CSU) or a Crisis Residential Unit (CRU)
- Participating in CARE Court
- In recovery and transitioning out of residential treatment for a co-occurring disorder
- Transitioning out of foster care

Completed referral forms should be submitted to the Sonoma County Department of Health Services (DHS), Behavioral Health Division (BHD) BHBH Coordinator for review and consideration at [add email or link here].
For Marin County Referrals:
Marin County Behavioral Health and Recovery Services (BHRS) manages its own referral waitlist and prioritizes referrals independently based on local needs and program criteria.
