Under contract with Alameda County Adult Protective Services (APS), support is provided to older adults and adults with disabilities with time-limited care management services.

Adult Protective Services Case Management (APSCM) for Alameda County

APSCM is a voluntary case management program specializing in stabilizing vulnerable adults in the least restrictive manner. It provides targeted case management in collaboration with the Alameda County Social Services Agency Adult Protective Services (APS) program to facilitate linkages to ongoing resources for older and dependent adults.

APSCM provides support to ensure older and dependent adults living in the five regions of Alameda County may continue to live in the community of their choice, with safety and supportive services.

Home Safe Case Management (HSCM)

Home Safe Case Management (HSCM) provides housing related intensive case management and housing stabilization for people served by Alameda County, Adult Protective Services (APS). We assist clients who are at imminent risk or are experiencing homelessness to obtain and preserve tenancy or receive a higher level of care when needed.

While assisting with housing navigation, HSCM also connects clients with linkage to community resources to support their ability to maintain housing when housing is acquired.
