Case Management for Older Adults and Adults with Disabilities
Care management services for older adults and adults (ages 18 and older) with disabilities who may be in need of brief counseling or access to more extensive mental health services. Our work with clients may include screening and assessments, treatment plans, and care coordination, both in the office and in the field as we offer community outreach and home visits. No fees are charged to participants.

Case Management for Alameda County
Case management, under contract with Alameda County Adult Protective Services (APS) to help help older adults and adults with disabilities resolve issues related to abuse and and to provide connection to services for individuals to thrive in the community. Ages 18 and older.
