An innovative residential transitional housing program located in Santa Rosa, Sonoma County, Crossroads to Hope (C2H) provides 6-month housing residencies, on-site case management services and programming for individuals with justice-involved and mental health backgrounds.
How Does C2H Support Residents?
Crossroads to Hope (C2H) is designed to help stabilize a person’s living situation while also providing supportive services onsite.
The aim of C2H is to help residents build a support network, access community resources, and establish long-term stable housing. The peer-led client centered program strives to provide a safe, understanding, and encouraging space.
Our staff are committed to providing personalized support and guidance. We believe in creating an atmosphere that encourages growth. We recognize that everyone has different needs and goals, and we are here to help facilitate those goals. Peer Providers led by a Program Manager will staff the program 24 hours per day, seven days per week.

Residents are pre-screened and identified through Sonoma County’s Mental Health Diversion program using an intensive evaluation to determine their needs. The assessment includes a violence risk review to ensure public safety.
Crossroads to Hope (C2H) is run out of a beautiful historical craftsman home built in 1915. It has welcoming common spaces, including a shared kitchen, a large front porch, and a back patio.
At full capacity, we can provide space for a total of 6 residents; 2 living in the main building and an additional 4 residents in the two double-occupancy bungalow cottages in the back of the property. Each resident has their own private room.
