Older Adult Intensive Case Management (ICM) and Full Service Partnership (FSP) programs

Providing direct support to adults ages 60 and older living with severe mental illness, our multidisciplinary, team-based mental health programs offer intensive wraparound outpatient services delivered in both the office and the community.

The goals of Felton Institute’s Older Adult Intensive Case Management (ICM) and Full Service Partnership (FSP) programs are to engage with, stabilize, improve the functioning of, and support the recovery of individuals with chronic mental health and co-occurring conditions (e.g. mental health and substance use, mental health and cognitive impairments) that result in severe functional impairments.

Who We Serve

We provide direct support to San Francisco residents, aged 60 years and older, living with a severe mental illness. Many of our clients are living on a fixed income, are insecurely housed, experiencing homelessness, and/or have co-occurring substance use disorders or health conditions.

Our services include:

  • Outreach
  • Psychiatric medication management
  • Individual and group rehabilitation and psychotherapy
  • Targeted case management (e.g. obtaining entitlements)
  • Collateral services
  • Crisis intervention
  • Substance use disorder intervention
  • Supportive housing
  • Peer support
  • Adjunctive wellness-recovery services, including linkage to needed health, housing, and human services

Felton Institute’s Adult ICM and FSP programs serve clients who are too acute to be supported in a standard office-based outpatient mental health setting and face health and safety risks resulting in the high use rates of psychiatric institutionalization, homelessness, and involvement with police and the forensic justice system.

Our programs provide an integrated recovery and treatment approach that includes working with the client’s family members, significant others, and support persons. We work from a client-centered philosophy that is strength-based, recovery-oriented and follows a harm reduction model.

Authorization for Older Adult ICM and FSP Levels of Care by the San Francisco Department of Public Health Behavioral Health Services is required.

Prospective clients must:

  • Live in San Francisco
  • Be 60 years and older
