Our multidisciplinary, team-based mental health program provides intensive wraparound outpatient services to young adults (ages 16-25) living with severe mental illness. Direct support is offered both in-office and in the community.
The goals of Felton Institute’s Transitional Age Youth Full-Service Partnership (TAY FSP) program are to engage with, stabilize, improve the functioning of, and support the recovery of young individuals with chronic mental health and co-occurring conditions (e.g., mental health and substance use, mental health and cognitive impairments) that result in severe functional impairments.
We provide direct support to San Francisco residents aged 16-25 who are living with a severe mental illness.
Many of our clients live on a fixed income, are insecurely housed or experiencing homelessness, and/or have co-occurring substance use disorders or health conditions.
Our services include:
- Outreach
- Psychiatric medication management
- Individual and group rehabilitation and psychotherapy
- Strengthening and collaborating with support networks to enhance client success
- Crisis intervention
- Substance use disorder intervention and linkage to treatment
Targeted case management, which includes:
- Assistance with obtaining entitlements
- Linkage to housing
- Connection to job training programs
Authorization for TAY FSP Levels of Care by the San Francisco Department of Public Health Behavioral Health Services is required.
Prospective clients must:
- Live in San Francisco
- Be aged 16-25
Medi-Cal and Medicare insurance are accepted.
