Mission to Motivate (M2M)

Based in Alameda County, the M2M Program provides services to clients involved in the criminal justice system with mild to moderate mental health conditions.

Felton’s Mission to Motivate (M2M) Program provides mental health services to justice-involved adults with mild to moderate mental health conditions who are eligible for AB 109 and AB 1950 funded services. These individuals may also have co-occurring disorders, food insecurity, experiences in homelessness, and/or limited treatment history.

Our mission is to motivate clients by providing support in removing barriers by utilizing resources that allow returning citizens to re-engage in the community. Felton makes it a priority to provide services to clients involved in the criminal justice system with mild to moderate level mental health conditions.

Our program conducts community outreach to develop awareness and build partnerships with resources for all of our clients.

The M2M program helps increase coordination among its service delivery systems; supports clients in navigating the transition of care to improve client wellness and community functioning, expands the interdisciplinary treatment team approach of planning of care to connect clients with short and long-term supportive services, and improves client outcomes to reduce recidivism and promote self-efficacy.

  • Not currently enrolled in a health care service agency

The Three-Phase Program Model includes stabilization, transition, and sustainability.

  1. Working with clients to meet immediate basic needs.
  2. Initiating connections and linkages to mental health, medical, and substance use services.
  3. Assisting with Medi-Cal enrollment and finding a medical home.
