
Designed to divert individuals experiencing behavioral health and substance use challenges into appropriate care and out of incarceration.
Felton Street Case Management, (FSCM) offers participants individualized case management and clinical linkages designed to meet the needs of low-level drug offenders who are often using substances and/or have mental illness. The program’s key components include: Case Management, Community Outreach (which includes mental health services and crisis intervention), Community Outreach and Engagement, toward having each participant meet the program objectives, and graduate from the program and successfully transition to a lower level of service and support.
The FSCM team Implements a harm-reduction philosophy, staff provide clinical case management, behavioral health and wellness interventions to support clients in addressing their substance use and/or mental health needs, and support recovery.
Team members work with residents referred in the city of San Francisco. Our services foster and encourage individuals to engage in positive, developmentally appropriate pathways, access a support structure that creates and maintains stability in their lives and helps prevent recidivism.
FSCM Case Managers follow the principles of harm reduction, trauma-informed care, and recovery and wellness in order to engage high-need adult clients in therapeutic interventions, link them with appropriate social services, and support them on their path to recovery.
